人造甜味剂对 2 型糖尿病合并肥胖患者代谢健康影响的系统

摘要

人造甜味剂作为无热量或低热量的糖替代品,在2型糖尿病合并肥胖患者中广泛使用。然而,其对这一特殊人群代谢健康的影响存在显著争议。本系统综述综合了近年来的临床研究和机制研究证据,系统评估人造甜味剂对血糖控制、体重管理、胰岛素敏感性及肠道微生态的影响。结果表明,人造甜味剂的代谢效应呈现“悖论性”:短期使用可能有助于热量控制和血糖管理,但长期使用与胰岛素抵抗加剧、肠道菌群紊乱及代谢综合征风险增加相关。值得注意的是,现有证据主要来源于一般人群或动物实验,针对2型糖尿病合并肥胖这一特殊人群的高质量随机对照试验仍十分有限。本综述提示,应重新审视人造甜味剂在该人群中的常规推荐,未来需要更多长期、前瞻性研究以建立循证指南。
