不同年龄结直肠癌患者血清CA19-9与CEA表达水平的差异分析
摘要
平差异，分析其与临床病理特征的关系及临床意义。方法：回顾性分析2022年1月至2026年1月我院收治的143例经病理
确诊的结直肠癌患者的临床资料。根据年龄分为青年组（<45岁，n=18）、中年组（45-74岁，n=92）和老年组（≥75岁，
n=33）。检测所有患者术前血清CA19-9与CEA水平，比较三组间的表达差异，并分析其与肿瘤部位、TNM分期、分化
程度及淋巴结转移的相关性。结果：老年组患者的血清CEA中位水平及阳性率（4.840 ng/mL，54.55%）均显著高于中
年组（3.481ng/mL，36.96%）和青年组（2.030ng/mL，27.78%），差异具有统计学意义（P<0.05）。青年组患者的血清
CA19-9中位水平（4.78 U/mL）均显著低于中年组（10.60 U/mL）和老年组（18.60U/mL），差异具有统计学意义（P<0.05）；
阳性率（>39.0 U/mL）无明显统计学差异（P＞0.05）。相关性分析显示，在老年组中，CA19-9及CEA高表达与晚期（III-IV
期）及淋巴结转移显著相关（P<0.05）。结论：老年结直肠癌患者血清CA19-9及CEA表达水平显著高于中老年患者，且
与更具侵袭性的临床病理特征存在一定相关性。CA19-9及CEA可能作为老年结直肠癌患者更具参考价值的肿瘤标志物，
对其病情评估、预后判断具有独特的提示意义。
