虚拟现实正念疗法改善共病重度焦虑的偏执型精神分裂症患者症状 1 例 ——案例报告与机制探讨

摘要

总结 1 例虚拟现实结合正念认知疗法(VR-MBCT)应用于 55 岁重度焦虑障碍患者的干预过程与效果。关键措施
包括:在药物治疗效果不佳并出现不良反应后,采用 VR-MBCT 进行系统干预,共 16 次,每周 2 次,每次 35 分钟。通过
沉浸式虚拟环境进行正念呼吸、身体扫描、肌肉松弛等训练,并结合焦虑自评量表(SAS)、汉密尔顿焦虑量表(HAMA)、
正念注意觉知量表(MAAS)、匹兹堡睡眠质量指数(PSQI)及震颤评定(TRS)进行阶段性评估。结果显示,干预 2 周
后患者焦虑症状及震颤明显缓解,4 周后基本控制,8 周后各项评分恢复至正常范围,睡眠质量显著改善。出院后一周随访
未见复发。本案例提示,VR-MBCT 可作为中老年重度焦虑患者,特别是对药物治疗不耐受或传统心理治疗参与困难者的
一种新颖、安全且有效的非药物辅助干预选择。
